I think you should bring it up. I know of therapists who lower their fees for some clients (my former one included) in cases of financial difficulty, etc., but don't know of any who charge more than their standard fee to a certain client. That seems unethical, IMO. I know you've been seeing him a long time - has your fee gone up in that time? Did you sign any paperwork in the beginning stating that you were responsible for whatever insurance didn't pay, etc., that might have listed his standard rate? Do you get an explanation of benefits form from your insurance that would have his charges on it?
Perhaps you can just ask him what his standard fee is. Unless he's charging you more now than he was in the beginning, I don't see how his fee could be any reflection on how he feels about you - he didn't know you in the beginning, so he couldn't have made that judgment call.
