Alcohol destroys brain cells in far greater numbers than MDMA. There is no clinical evidence (that I know of) that MDMA is likely to have bad long term effects, if taken infrequently. This is not to say that researchers won't find long term ill effects from infrequent but long term use. I doubt they will though.
Personally I wouldn't touch MDMA with a 10 foot pole. Anything with a psychedelic effect scares the daylights out of me. But, for a good many people that I know infrequent use of X is beneficial for their mental health. Perhaps for some people X is the psychiatric equivalent of a gasoline additive you use every 6 months to unclog your fuel injectors.
MDMA is just another in a long line of drugs made illicit because they make non-ill people feel better than normal.
