It's a direct, receptor-stabilizing inhibitory effect. Niacinamide "raises the threshold" for mast cell histamine release. All the relevant research was done in Eastern Europe, during the Cold War. They had a totally different philosophy over there. E.g., Picamilon is a result of the research into niacinamide modulating the GABA(A) receptor.
