I don't think it is about alter personalities...
I think it is just that methamphetamine, typically, does result in intense feelings of paranoia (fight / flight response) at high doses / over longer time periods.
The way that alcohol results in motor co-ordination problems and vomiting at high doses / over longer time periods.
I know that some people get violent with alcohol... Some other people get weepy with alcohol... Different people have different responses with it, depending a bit on their personality etc etc. But I have only ever heard of people developing paranoid hallucinations (typically auditory) and delusions with extended use of methamphetamine.
It is true what you say about sleep deprivation... Extended use of methamphetamine does get all mixed up with effects of sleep deprivation...
