I do think it varies with each T as Falls said. Both my T and my daughter's T give me a statement at the end of each month. But I do the paperwork for the insurance, i.e. I pay the full amount to both therapist and get my portion back from the insurance company.
Finding the right therapist for you will be the bigger job. I would tell the therapist you are interviewing your past experience, both the good, bad and ugly.
