That German study sounds very interesting. Even if trimipramine is statistically inferior to Haldol and Thorazine, it would be a no-brainer to try trimipramine first and avoid EPS - as long as a individual's state doesn't require an immediate intervention. Of course, you might be able to introduce both drugs simultaneously in this case, and later try to withdraw the antipsychotic. I don't think there is much risk of being unable to recapture the antipsychotic under these circumstances. The antipsychotic would be tapered. As soon as a deterioration presents, the dosage of the AP would immediately be returned to its effective dosage.
Just a thought. I have no facts to support this idea.
