Public drinking water has been fluoridated since the late 60s or so in the US. Zillions of public health studies have been done. There was widespread opposition at first and all kinds of conspiracy theories about big government trying to poison people and on and on. Studies over the years have found no ill effects at all, but a huge reduction in tooth decay. When I was a child (prior to fluoride), I would go to the dentist each year and need 10 or more cavities filled. I didn't eat a lot of sweets. My teeth were just rotten and this was common in my generation. Now this is very rare. My daughter had maybe 5 cavities in her entire life.
Fluoride has virtually eliminated severe problems with tooth decay in the US.
