The selection of desipramine makes a great deal of sense, especially because it is often helpful when treating ADHD and avoids weight-gain. However, without empirical evidence suggesting otherwise, I don't think one can conclude that (Zoloft or Lexapro) plus desipramine is universally more effective than Effexor at treating depression, despite theoretical considerations. Such a combination is certainly worth a try, though. It is a good suggestion. I would just offer that it might make more sense to add desipramine to Effexor first, and then switch from Effexor to an SSRI if necessary. If this fails, it is reasonable to switch from the SSRI to Parnate while retaining the desipramine, depending on the degree of treatment resistance.
