I have to tell your the real facts of writing these reports and that I have done this with previous doctors and they discarded them and took nothing into account of my statements when I needed to inform them and eventually discharged me because there where complications they did not know what to do. my previous doctor Dr. XXX was a good doctor but due to my substance abuse, he discharged me and I was left in such distress of what just happened, no one listened and thought I was full of b*llsh*t.

Maybe try: discharged me and I was left in such distress that I felt no one was listening to me or believing me. (BS=not good)
__________________I'm not a big fan of writing to doctors. Just like online communication, words can be misinterpreted.
I know that you're trying to be heard this way because it's hard for you to verbalize it. But have someone, besides me, read it to make sure that you're getting your main points across while using as few words as possible.
Better yet, write your main points down and read those to your doc. I do that pretty often for the med side of things.
