I know how you feel. You know sleep will make everything much better but you can't get to sleep and/or stay asleep. You are right, your high cortisol levels in the evening are unusual (normally cortisol is highest in the morning) and would make sleep more elusive.
I've found a lot of relief with Seroquel (an Rx that ain't cheap but will help you sleep). Also, a new antidepressant is coming out fairly soon that should help called Valdoxan. Watch for it and ask your doc. You may want to make sure you seeing a doctor who understands and is interested in helping you with the high cortisol / HPA balancing.
Unfortunately, with regard to HPA balancing as I understand it, we are just learning about dysfunctions of the HPA axis and have yet to come up with a treatment that will move it toward balance and normal function. Stay tuned...
Finally, one natural product which will help reduce cortisol is L-Theanine. You can find it at most health food stores or online. It can help you get back to sleep when you wake up in the night.
