I know what you mean. I have some dx's that could cause serious career/educational problems if they weren't kept secret.
I think you should talk about confidentiality issues with your therapist and psychiatrist. You should be able to request that they not formally dx you with anything, and avoid making written notes that could be used against you. Hopefully, you have (or can find) doctors that understand these issues and will help you with your symptoms without labeling you.
For example, when I suffered an episode of paranoia my psychiatrist/therapist called in a prescription of an antipsychotic labeled "take one or two tablets as needed FOR ANXIETY."
