The symptoms you are describing sound so dramatic that I would strongly suggest that you see a neurologist immediately. You should be sure to rule out any serious organic brain problem.
Your story has a lot of resonance for me, since I was a physics major at a leading university when I first became seriously mentally ill. As my condition deteriorated, I eventually had to drop out.
Fortunately you sound exceptionally pro-active and well-informed, and I am sure that you will beat this. Many of the symptoms you are describing could be the result of sleep deprevation--however I also suffered terrible sleep deprivation in college without noticing such dramatic and persistent symptoms.
I'm glad that you are getting treatment for your depression and anxiety. Hopefully your sleeping patterns will improve as well.
I still have the nagging feeling that something else could be up. Based on the little I know, your symptoms sound like they might have a neurological basis. I think you should see a good neurologist as soon as possible.
