Is dosage based on size (height and weight?)

I read a bunch on this subject because I was curious if I should try 15 or 20mg to increase my "benefits" from taking Lex. My DR refused to go along with it and said if 10mg wasn't working I should probably try a different med. Have been on Lex around 5 mos.

The worst was chest pain/pressure in the first two weeks. Now the main SE are slight dizziness and feeling sleepy (constant yawning). Also in the last week I've had new SE including jaw clenching and headaches (probably from the jaw clenching). Strangely enough, I find it encouraging that the SE have changed over these past six weeks. It makes me feel that changes are still occuring with use of the Lex and there is still a possibility that I will get some benefit when I get to week 8 or 9. Not exactly scientific thinking but whatever it takes to keep me going............the idea of having to start over with a new med, that may or may not work after months of use, is that much more depressing.
