I'm seeing a lot of mild hypernatremia in my patients on Depakote ER (divalproex SODIUM). Do you simply get an excess of sodium when, in the gut, it dissociates into the valproate ion? Anything to worry about if asymptomatic? Is this something I should already know and just haven't seen it before?
Anyone see any utility in checking urine and plasma osmolality and urine sodium?

1. is the development rapid?
2. have you tested any of them, do that have normal total-body sodium? or are you seeing a notable difference? higher or lower than norms?
3. are they able to just do an oral water replacement? or do they over-do the intake?i would look at the osmolarity thresholds vs sodium as asymptomatic may have normal or close to normal levels.
