I've been on most of the serotonin reuptake inhibitors, Effexor, Cymbalta, Wellbutrin, Nardil, Namenda, and most of the atypical antipsychotics. I AM the definition of treatment resistant. If it's not the serotonin, the norepinephrine, the dopamine, the GABA, the glutamate..then what is it? Pot helps, but don't think I advocate it in any way. I don't know why it helps, either because it targets one of the pathways involved in OCD or simply because I'm too relaxed to count to 7 repeatedly (one of my compulsions). Opiates are right up there with pot, but that's like playing with fire..same goes for alcohol.

Ok, according to wikipedia the "central dysfunction of OCD involves the receptors nk1, non-NMDA glutamate receptors, and NMDA". I kind of figured we were on to something with the whole glutamate dysfunction hypothesis. I responded to the NMDA glutamate receptor antagonist Namenda, but only at doses double that which had been studied. nk stands for neurokinin apparently...a neurotransmitter, substance P, binds to these nk receptors. There's a drug, Emend (Arepitant), that IS a substance P inhibitor in that it blocks nk1 receptors. It's prescribed to patients receiving chemotherapy..for nausea. Yeah, I don't know about that one.

I thought Luvox and Anafranil were two of the drugs used for OCD...did you try those? I don't have OCD but I'm sure the other posters can give you recommendations for what has helped them...Good luck...SG

Its suspected that cortisol plays a part in it...as it does with most anxiety/panic/agoraphobia/etc.

I will leave it to the other posters to discuss the causes of OCD--it has been extensively researched. On a practical level it responds quite well to aggressive CBT (specifically ERP--Exposure Response Prevention) and high doses of SSRIs (Prozac, Luvox and Paxil in particular). Theoretically it should respond to other SSRIs but I know many experienced clionicians appreciate the older ones for OCD. Good Luck fca

I think luvox helps as I am not as compulsive anymore and that is on low doses which I have to raise when my thyroid results are in. Love Phillipa

Oh the doc checked cortisol levels and they are normal. Love Phillipa ps OCD not bad it's the anxiety

I agree that POT helps, so does alcohol.
