I called the Medical Records dept of the hospital and inquired about the procedure for obtaining my record. They sent me a form which I filled out and returned. It took a few weeks, but I got my complete record in the mail. And, yes, I must say it's disturbing to read. Even after all this time, I still get very nervous every time I read it; and I don't know why I read it as often as I do (strange).
