I'm sorry you're feeling terrible right now. I have struggled with difficult PMS for years and I've found a few things that help. For a long time I was applying progesterone cream to the undersides of my arms twice a day. I use the natural soy stuff. It really helped my cravings and mood swings. Just follow the directions on the jar. I also take a B complex vitamin. It's important to take all 3 B vitamins together or else your body gets depleted of the other 2 (if you're only taking B6, for example). But the B complex really helps with cramps.
When I told my MD what I was doing, he looked at me strangely. But it's worked for me and several other people I know. Lastly, pleast take good care of yourself. It's so important to have a support structure. Do you have a therapist? That can be a great start. But there are great people here on Babble that can lend support too.
