Racing thoughts

What is the difference between the racing thoughts one gets from anxiety and the racing thoughts one gets in a manic or hypomanic phase?

My whole life is racing thoughts. That is all my brain is filled with, all the time. If someone were to ask me what one of those thoughts were about, I wouldn't be able to tell them - it's just this feeling of constant background noise. Antidepressants either make these racing thoughts worse or make me more depressed or both, it seems like. However, I know that antidepressants also cause an increase in anxiety upon start-up. So, I guess what I am asking is how can I tell if the racing thoughts I get from being unmedicated & the ones I get from going on ADs is just from severe anxiety or from a possible hypomanic phase?

I just can't understand why nothing seems to be getting better even after lots of therapy and all of the SSRIs/SNRIs/a really short trial of one MAOI. Am I not giving them enough time or are ADs not for me anymore?

PS - I apologize if I put this is in the wrong section. It has to do mainly with psychology I think although medication is a part of my problem.
