I also mostly noted from the original poster the huge disparity between concierge/self-pay-at-full-rate medical service vs the lower rungs. (These would have various rungs for different levels of insurance, with Tricare, Medicare, and Medicaid being lower than commercial insurance, followed by uninsured-and-unable-to-pay.)
