In my case, I could say physical therapy, because I'm known to have a couple of serious physical problems. Of course, I also don't work, so don't need to say anything. I'm not sure I'd disclose therapy, but tend to think I might. There's also the "Life Coach" option, if you want to try that, but then I suspect you'd get a lot more questions.
I guess it depends on your coworkers, and your relationship with them -- the first couple work well enough if you have a good rapport, because they get the idea that you don't want to talk about it, but you've also made it a joke.
