Yes I think alot of them just take short notes and dictate later.. Alot of clients don't like the T writing things down while they are talking to them. I am sure some are more detailed than others. The ones I have seen are detailed enough for you to figure out what is going on..Because of my job I have seen more than a few of their notes. I try not to read them at all if there is not info there I need.
