Most therapist will assess the thoughts against possible actions. Talking about those thoughts is one way of asking for help. But they have to take what you say seriously. So if you are expressing wishes to harm yourself, action of some kind, even if it is "just" a contract with your Therapist is usually taken.

My therapist encourages expression of the thoughts. He thinks they get bigger if they are kept a secret. And he often equates them with different parts of me that wish the pain would end, not that my life would end.
