Well, if you had a semi-good experience on Effexor, Cymbalta is probably worth trying again. Of the two, Cymbalta was a kinder, gentler Effexor for me: far fewer side effects, and much easier to stop.
Based on your list, I'd say you do have a few options out there, although I can certainly understand being discouraged after hearing that from a pdoc.
Before I actually offer anything like suggestions, what sorts of symptoms do you have? Diagnosis is all well and good, but I don't know how helpful it really is for figuring out what to try next. Sometimes, symptom lists are more helpful for me. The DSM lists depression, but it can have contradictory symptoms: weight loss/weight gain; over-sleeping/under-sleeping; etc. So, knowing that your own personal form of depression includes the following symptoms, but not these others over here, might be helpful for you and for your pdoc.
One of the interesting things I notice in your list is that two related serotinergic medications were helpful for you, but the other serotinergic medications you've tried were Not Good. That leads me to wonder what sorts of problems the others caused, and what effects were good for you.
