You know because you go to your doctor, who does tests and makes the diagnosis. That really is the only way.
Type I diabetes, though, is caused by the pancreas ceasing to produce adequate insulin in response to food you've eaten. It's probably an autoimmune type thing, but it's generally not diagnosed in people over about 20. Type I used to be called "Insulin Dependent Diabetes Mellitus," because it requires injections of insulin.
Type II diabetes is a different matter, and it's te type that is usually diagnosed in adults. Rather than being caused by a lack of insulin, it's caused by insulin resistance -- your body doesn't seem to be able to use the insulin which is produced. It can often be controlled by diet and exercise, but it may require a medication to help your body use the insulin that it's producing.
Phillipa just offered a list of some symptoms that might suggest diabetes, but it you think you have it, you'll have to see a doctor. If you are bound and determined to figure it out for yourself, head down to Walgreens and buy a glucometer, a bunch of test strips, and a lancet, and test your blood sugar before and after every meal. Usually, test about half an hour after, then about three hours later. See what happens to your sugar levels. Keep a journal of what you eat, when, and what your levels are. After a week or two, you should have an idea of how you're doing.
Oh, and while your doctor may not have come across this, some studies have shown that Effexor can affect blood glucose levels.
