Do especially go to the ER if you get a very irregular heartbeat and a very high fever (like 104 or higher).I think the fever is a critical issue, but I would put the threshold for seeking emergency care much lower, at 100 (absent infection), if it matches the heart disturbance in time (i.e. both are flaring simultaneously).
