OK, white spots on nails mean a zinc definiency and right there you've got a problem. Internal pressure is a feeling like you're going to blow up - a sense of anger or urgency. Feels like a hot red boiling vapor is steaming out and expanding inside you. It can manifest in different forms - a meltdown, angry blowup, violent outburst or feelings. It gets worse with any kind of stress and the anxiety is so uncomfortable that alcohol is #1 favorite self-medicating choice for the releaf of stress. In fact, they recognize the need for benzos or some strong axiolytic when stopping alcohol because not replacing the stress reliever can go very hard for for pyrolurics.
Briefly, kryptopyroles are producted in the manufacture of hemeglobin. Pyroluria, a metabolic disorder, causes these pyrrole groups to strip zinc and Vitamin B-6 out of the bloodstream, both of which are cofactors in the synthesis of serotonin. Zinc is so important to so many other processes as well, that if you have fingernail white spots, you almost certainly have a zinc deficiency and should be very intent on getting that fixed.
If you want to get tested in pyroluria, the test is cheap, $40. It's finding a doctor that knows about this stuff and to interpret it, and prescribe a nutritional regimen. The sites will give you ideas on what you'd typically need, but the real life story is usually different, and it's important to find a professional to work with you. This lab:

They also have a metabolic psychiatrist at their health center and they have a whole medical workup program. I long to have this done!
There are other metabolic disorders they look at too for mood disorders. This is a completely different approach than our mainstream pdocs. I personally don't know of anyone with this disorder but my doc says she's seen quite a few who have been helped tremendously. Your symptoms jumped out at me as possibly pyroluria.
