My doc says the over the counter melatonin found in the vitamin section is very hit and miss. The best way is to get a prescription to take to a compounding chemist. Expensive though if not covered by insurance. I pay $1 for each 3mg capsule in Aus.
I started at 3mg, worked up to 6mg but didn't notice any improvement between 3mg and 6mg so went back to 3mg. Very helpful for me. The literature has melatonin prescribed up to 10mg safely. Haven't noticed any side effects personally.
You could also try the non-sustained release seroquel. Not an AD though. Can help with anxiety.
