I think the higher does are for OCD symptoms. I took it for anxiety but I know what you mean by the symptoms your describing. It is supposed to really work for them.
I have some OCD and paxil definitely helps me.

Diagnosis: depression and anxiety
Current meds: paxil 20mg., desiprimine 20mg., clonazepam .5mg.
Of meds tried, nortriptyline worked the best but caused major weight gain so d/c
