Hey RJ, sounds terrible. I used to have that kind of thinking-without-acting on suicide a lot. Repetitive, intrusive thoughts. I don't know how old you are or what meds you are on, but there are meds that can help that sort of thinking so yeah you really need a doctor of your own, not someone who is essentially your mother's doctor and is treating _her_ for the "disease" of having a troubled and troubling son. Mental illness is as much social as it is neurochemical and your social / family situation sounds bad. What are your options for changing docs? For putting some distance between you and your mother? The main thing to keep in mind while you look for ways to make changes is that things are not hopeless -- you can get yourself stable.
