It can be complicated to find out especially in the elderly? My first thought is cancer or cardiac/pulmonary problems because that's what I see mostly. But then I found this nifty mnemonic online geared toward weight loss in the elderly. I think some of it could apply to any age group. I hope this helps your friend.
