Augmenting Zoloft with Straterra might be a good option. Straterra is FDA-approved for ADHD, but was originally developed as an antidepressant. It inhibits norepinephrine reuptake, which would work well in combination with Zoloft which inhibits serotonin reuptake. Another option would be switching to a dual reuptake inhibitor such as Effexor, Cymbalta, or Pristiq. These medications inhibit reuptake of both serotonin and norepinephrine.

Another option would be a mood stabilizer such as Lamictal or Lithium. When you say "poop out," which symptoms are returning?

Every time I go up on the Zoloft, I feel better for about a week, then it starts to poop out. I know this is all pretty much a crapshoot, but has anyone had success augmenting Zoloft with other meds, or would anyone have any suggestions about what to try? Many, many thanks.

I don't think the Zoloft has done much for the anxiety; I think it's primarily the depression it helps. A few meds other people have mentioned to try as augmentation agents are Strattera, Memantine, Pamelor, and Provigil.
