Dear Dr. Kramer:
I am posting for my father - 72 yrs old, on maintenance low dose steroids for an allergic skin condition, no other chronic medical conditions, first episode of major depression and anxiety since about 3 months, initially placed on citalopram 20 mg for 4 weeks, no response; switched to lexapro 20 mg, minimal response in 3 weeks; the psychiatrist added risperidone, intially 1 mg bid, increased to 1 mg in am and 2 mg in PM. Response to the first dose of 1 mg of risperdal was salutory, but reverted back after about a day; not much of a response with even the dose increase. Was the initial response indicative of further need for augmentation with another antidepressant? Should the dose of lexapro be increased further? Will adding bupropion/ buspirone help? Will increasing the dose of risperidone help any?
Is there any association of low dose steroid maintenance therapy with development of late life depression? I apologize for the number of questions...am just really perplexed at the moment.
