I got a consult done once, with a medical professor who was an MAOI expert, and (despite labeling restrictions) he told me firmly that Remeron had been found safe to use with MAOIs. In my written consult, he specifically recommended it in combination with them as a useful and powerful augmentation strategy. The dosage of the Remeron need not be restricted, either.

I have read that Remeron can be used acutely to treat serotonin syndrome, I presume because of its receptor antagonist effects (?). This may be part of why it's not dangerous in terms of hyperpyretic crises. The same doctor considers most tricyclics (NOT clomipromine, possibly not trimipramine) a possibility to use in combination with MAOIs, as long as it is monitored carefully. I have found at least one article recommending the same (despite all the labeling warnings).
