Emsam - highest dose?

Just asking this again for those who didn't see my question above. Anyone know how high you can push emsam to? I mean, above 12mg a day. I saw where Monte said he has heard of 18mg. I have been on 12 mg now for over 2 months with minimal success and would like to go higher.
