That's quite a cocktail you've got there. Doesn't it seem like such an injustice to take so many drugs for so little improvement? I'm in the same boat. I would like to suggest that you place Keppra 1000-2000mg on your list of medications to try. It can simply be added to what you are already taking. There is little study of its use in depression, but there do exist anecdotal reports of its helping with rapid-cycling bipolar disorder and can have antidepressant effects in general.
I'm sure you've already taken into consideration the addition of Wellbutrin 300mg/day.
Depression takes its toll on self-esteem, despite attempts at positive thinking on the part of the sufferer. Depression is a crafty liar. You might perhaps think of doing some CBT (cognitive-behavioral therapy) to help isolate unrealistic automatic thoughts of self-reproach and become more aware of your positive attributes through the application of a more balanced perception of you on the part of people who know you as well as exercising your own logic to evaluate the validity of your negative thoughts and conclusions.
