I have life-long, treatment-resistant dysthymia (which can easily slide into double depression, especially in the winter) and moderate social phobia /w inattentive ADD, although I wonder if the inattentive ADD just really isn't some kind of reward deficiency syndrome that goes along with dysthymia.
I get 100% remission from a combination of an MAOI and buprenorphine. I was on tranylcypromine 200mg/day alone, which was working very well for the social phobia and OK for the dysthymia, but the insomnia was driving me crazy. Also, my hookup for raw tranylcypromine powder, which enabled me to take that much, raised their price such that I could no longer afford it. So, I switched to Nardil today. So far, so good. Much calmer, a little tired, definitely hungrier, but no bad S/E yet (knock on wood!)
When I added the buprenorphine, I felt like I was reborn. That dysthymic "blah, why bother" and inability to follow through on things no matter how hard I tried--even something as menial as cleaning my room--vanished almost overnight when I hit 16mg/day buprenorphine.
