Ketoconazole is a systemic drug that is sometimes used to treat candidiasis. It has also shown some benefit in depression. Although it is not completely clear how it works, in addition to its antifungal action, ketoconazole inhibits the secretion of cortisol from the adrenal glands. More recent theories to explain depression focus on the system that controls cortisol and posits an abnormality in its regulation. Known as the HPA axis (hypothalamo-pituitary-adrenal), it seems to be overactive in many people with depression. There is much debate over this topic.
