If it is *word-finding* difficulties when you are trying to speak-it might be med-related. SSRI's are well-known to cause this trouble for some people. I've had trouble with that before. AD's that boost up NE (desipramine, Straterra, etc.) seemed to help me. If you are having trouble understanding speech (listening-trouble discerning direction sounds come from), that could be CAPD (Central Auditory Processing Disorder). That has to do with problems in your temporal lobes processing your sense data from your ears and coding it.
