Dave, docs--like all of us--have impressions. Usually the longer you see the same doctor, the better he can get to know you and make accurate diagnosis and develop a treatment plan with you.
It's just, like lawyers, they are only “practicing" the art of medicine. This allows for tremendous leeway as opposed to them having to be perfect constantly.
Also, on different days you may "present" differently. I have 2 primary diagnoses’ and others that just rear there heads at times like those games at the carnival where you hit their heads with mallets when they pop up and you get to win cool stuff and amaze your friends.
ECT is one of the most effective treatments for treatment resistant depressives. Lithium is the "gold standard" of mood stabilizers or augmenting agents with anti depressants. I don't know much about maoi's as I don't think I could ever handle the side effects and dietary restrictions, so sorry about that part.
