Most people don't take 90mg, regardless of what you read on this site. Most people feel the full effects @60mg, you shouldn't be "aiming" for a specific dose.

As far as the hospital thing, I think what they are saying is that, typically, those ill enough to be in a hospital, may need 90mg to get relief from their depression, etc.
