Hi Geno,

I havent tried seroquel, but if it is anything like
zyprexa, it should have a pretty strong anti-anxiety effect
and a sleep enhancing effect. I found zyprexa to help my
sleep as well as Remeron. Zyprexa was MUCH better for anxiety.

Remeron may actually make people anxious (although the
antihistamine still helps with sleep). It made me feel a bit
jittery during the day but helped me sleep at night.
I'm not sure if its the best medicine for someone with
panic attacks. I don't have panic attacks but I felt
like my heart was racing a bit, I believe at the higher
doses the noradrenergic effect may do this. Others with
panic who have tried Remeron can comment.

Remeron increases the firing rate in a critical
noradrenergic area of the brain called the "locus
coeruleus" via its noradrenergic mechanism (it
blocks a feedback receptor that inhibits norepinephrine
firing called the alpha-2 receptor). The locus
coeruleus is a key area of the brain associated
with stress, it shoots faster when we are nervous.
Most ADS that enhance norepinephrine are
norepinephrine reuptake inhibitors. They prevent
norepinphrine from being recycled from the synapse
back into the neuron, leaving more norepinephrine
in the synaptic cleft. Over time they tend to
desensitize the "alpha-2" receptor and actually
slow down the firing rate of the neurons while
incresing the noradrenaline in the synapse by
blocking the reuptake recycle mechanism.
So Remeron has a see-saw battle for anxiety;
it increases the noradrenaline stress mechanism
but it blocks a serotonin stress receptor.
Anyways its generally noted that the noradrenaline
mechanism used can standalone elicit panic attacks.
(Yohimibine, a semi-selective alpha-2 antagonist can
elicit panic attacks).

Zyprexa and (maybe Seroquel) is usually an
excellent adjunct for anxiety and sleep disorders
as you may suspect.

Neurontin is helpful too and worth giving a try.
