"These cases of apparent adverse effects in four adolescents receiving venlafaxine manifesting marked mental status changes consistent with delirium and tachycardia after snorting cocaine highlight a drug interaction between this combination. The occurrence of four cases in one practice highlight the importance of identifying the risks of this combination.
The occurrence of a toxic, confusional state in these adolescents who were undergoing venlafaxine therapy and who used cocaine is consistent with reports of states of delirium and nonspecific confusion in cocaine users (Thomas, 1993) [27]. Hence, the achievement of delirium with relatively low doses of cocaine signals a potential drug interaction. For instance, the active compound in cocaine is metabolized by similar hepatic mitochondrial enzymes as venlafaxine (Gilman et al., 1990) [12]. An interaction with venlafaxine resulted in an elevation in serum and CNS levels of cocaine by reducing its metabolism (Gilman et al., 1990) [12}."
