This disturbed me too. I'm also bothered by the possibility that people are still posting under multiple aliases.
But anyway, on to Jeff's questions and comments...
I actually don't get "runner's high" or other experiences that are believed to involve the endogenous opioid system. I've tried intense physical exercise a number of times (both when I was depressed -- if you can imagine it! -- and otherwise); it just doesn't work. So I think that whatever is wrong with me probably is not exactly the same as your problem. I hope I can be of help to you nonetheless.
Have you ever taken opioids of any sort, and if so what was your reaction to them?
Dr. Bodkin (primary author of that buprenorphine article we've all read < g >) is still at McLean; he was the doctor who originally suggested this treatment to me, and my pdoc consulted with him when we decided to try it. I don't know his phone number there, but the main number is 617 855 2000. I don't think that he's seeing new patients (he's focussed mainly on research, last I heard), but he might be able to make a referral, perhaps to someone who's nearer to home (Massachusetts is rather out of your way).
I hope this is of some help to you.
