Sal,

If you're disturbed by the lack of RCTs supporting (or refuting) the efficacy of Ultram as an AD, then you should by all means feel free to provide the funding for one. This study should be conducted by an impartial researcher not affiliated with any drug company (e.g., or government agency.

You regularly extoll the virtues of dozens of other unorthodox remedies -- some of them well supported, others not. It hardly seems reasonable that others should be expected not to share their knowledge, or that their experiences should be considered less valid than yours (with, say, pindolol).

I took a look at the two citations you provided. I think many people are reluctant to give credence to animal models, but the fact is, they can be highly predictive of antidepressant activities in humans. We're not as different from rodents as we like to think.

Re the second of your cited articles: this is one of many papers that discuss the (enantiospecific) monoamine reuptake inhibiting effects of tramadol. I'm not clear on how these effects compare to those of SRI/NRI antidepressants, although I'm sure we could find out easily enough.

Incidentally, you missed a few:

Shapira NA, Verduin ML, DeGraw JD. Treatment of refractory major depression with tramadol monotherapy. J Clin Psychiatry. 2001 Mar;62(3):205-6.

Spencer C. The efficacy of intramuscular tramadol as a rapid-onset antidepressant. Aust N Z J Psychiatry. 2000 Dec;34(6):1032-3.

And on a related note:

Goldsmith TB, Shapira NA, Keck PE Jr. Rapid remission of OCD with tramadol hydrochloride. Am J Psychiatry. 1999 Apr;156(4):660-1.

(This is just what I found through a very superficial and cursory search.)

BTW, do you think that all research efforts (let alone clinical cases) result in publications?

-elizabeth
