Theoretically you are right about Flupenthixol. In a no. of double-blind studies it was found to be of 'equivalent' efficacy to ami/nortriptylene, @ low doses.
However few studies have been done for severe depression (true test of AD) & most of the benefits are seen in mixed anxiety-depressive states.
The theory behind this is that the traditional neuroleptics reduce depression scores chiefly through reducing anxiety (a big component of the HRSD). A less anxious person will likely feel better. This has been my experience with Sulpiride.
My pdoc refused to prescribe Flupenthixol a while back due to unacceptable potential risk for developing Tardive Dyskinesia.
The newer APs supposedly carry much less risk for this, & through actions on receptors other than Dopamine, are better all-round AD augmentors. Recent studies have noted synergy btwn SSRIs (ie Celexa) & the likes of Risperdal & Zyprexa.
