According to the URL James provided, duloxetine is only now being tested on a small number of both healthy and ill individuals (phase II) to establish whether it is safe and shows enough efficacy to advance to large scale clinical trials (phase III). In the mean time, superficially similar antidepressant combinations that produce combined serotonin and norepinephrine reuptake inhibition. One can either try Effexor or combine an SSRI with desipramine or reboxetine (available in other countries). For me, I still have more faith in desipramine, a tricyclic antidepressant, than reboxetine. It is a proven product. If you believe yourself very treatment resistant, I read that Effexor and Remeron in combination is considered to be a "big-gun" combination. I would try it, except that Remeron didn't treat me very well the first time I tried it alone.
I think duloxetine is a drug to keep an eye on. I don't think it would be approved any sooner that two years from now. Perhaps someone else can comment on the length of time it takes to get a drug to market once phase III trials begin.
