Tricyclics are not as likely as SSRIs to "poop-out" and stop working. However, as a layman, I would not be insistent that this cannot happen with dothiepin. I don't know of any antidepressant that is without the risk of depression break-through. Perhaps you simply need to increase the dosage. If this doesn't help, I would not worry. It does not sound like you are treatment-resistant, so I think you will be successful hunting for a new drug.
Have there been any psychological stresses recently that may account for medication break-through?
