it has been almost 5 years of trying reuptake inhibitor after reuptake inhibitor with zero benefit. 98% of psych meds ive tried have been like sugar pills for me. ive currently been taking adderall, which im not even sure has been beneficial at all the past few months, even after taking a break from it or trying a higher dose. and when it was helping, it was just just a very slight, but greatly appreciated benefit.
ive tried every class of todays typical "go to" meds. multiple meds in each class, as well. as well as lithium, ketamine and ECT.
anyways, i have another pdoc appointment tommorrow. and @ my laat appt with him he named 2 SSRI's (since ive tried every other one with zero effect) he'd consider if my current treatment didnt help. i just said "okay, cool" but i think thats a ridiculous idea. it seems most Pdocs feel theyve done their duty by sending you out the door with just about any of todays safer psych meds that are a bit safer than older meds. so all i can think of is asking to try an MAOI, or to just go home with no RX at all.
i mentioned at my last appt i always have a "better off dead" feeling. so maybe he would prefer to prescribe an MAOI, than for me to go home with nothing. idk the laws exactly, but he might be held liable if the worst were to happen (which i dont intend to do).

MAOI kicked off a mixture of addiction and psychosis for me.
You may not have the same reaction but some people do.
You may be one of them.

just got Emsam prescribed. and my pdoc said he#d be willing to prescribe an oral MAOI in the future if Emsan didnt help and i brought in a list of dietary guidelines that i wrote down, to show i know which foods to avoid. this was just my 2nd appointment with him, so i wasnt sure how afraid of MAOI's as my past pdocs. i'll be starting it in 2 weeks :)

no, no concern aboit bipolar. my mood/energy/motivation stays between flat and extremely flat. im just going to pay close attention to how emsam effects me since ive never taken an maoi before. tho emsam has been found to be safer, especially at lower doses, regarding food interactions compared to oral maoi's.
also, im thinking the pdoc im seeing now is my favorite out of about 6 ive seen in the past 5 years. hes not afraid of his own shadow to prescribe certain meds that can be risky. seems to be well versed in mental health treatment, with a logical view of it. and so far, i havent had to repeat something we discussed at the last appointnent, or even 5 minutes earlier in the appointment, like ive had to with every other pdoc ive seen.

IIn the interest of an open trusting two way relationship, and considering the high expense to you the customer, yes, you should speak your mind.
SSRI has already proven wrong for you. In the real world, my journeys suggest SSRIs fall way short of expectations most of the time. Drugs that seem to get people living again better than SSRIs seem to me to be ones that impact DA and NE. The particular mechanism is impossible to predict. But I think narrowing it down to those two increases odds of success.Choices would include MAOIs. Or SSRI + TCA. Or anything with AP. Or stimulants. After trying all the ADs, APs, and mood stabilizers, and many herbs, the supplement that helped me the most was Ritalin...the one no likely to be prescribed, right?
Longer term, clean up the diet and the immediate environment. Toxins are the issue. Messing with the brain big time. Lots of issues with some common foods, molds, common household chemicals, and unsuspected infections. All have two things in common....toxin accumulation....psychiatric symptoms.
