Mirtazapine seems to be pretty in vogue when it comes to adding something for depression (I've seen it suggested dozens of times, perhaps only second to MAOIs in recommendations.) I know it's technically contraindicated with TCAs (for SS, if I recall; maybe more.) Would it be a big deal to combine a small dose with 150 mg of Nortriptyline? Has anyone ever done this?

Not sure about wether the combination is safe - I cant see any obvious interactions, and seetonin syndrome would be exceptionalyunlikely.
That said, I'm not sure that the combination is especialy rational, it might make more sense to take nortriptyline with Lexapro, Celexa or Zoloft (ie, an SSRI) so that you're hitting both noradrenalin and seretonin.
Of course, provided your doctor says its safe, you dont have anything to lose by trying it.

The only potential interaction I can think of would be increased heart rate (or blood pressure). Nortriptyline is a strong NRI and mirtazapine enhances norepinephrine release.
Mirtazapine isn't really contraindicated with a TCA and It can be usually safely combined with SSRIs.
