Stopped Clomipramine, now unstable. Help!

Posted by Phidippus on November 23, 2011, at 18:44:18

I started Viibryd about a month ago, while I was taking Clomipramine. As I increased the dose of the Viibryd, I lowered the dose of the Clomipramine. Just last week I stopped the Clomipramine while remaining on the Viibryd at 40 mg.

I have experienced no change in my OCD symptoms, however I have become extremely anxious and irritable, with some depressive thoughts. I don't know what to make of this.

dx: adhd/bp nos/ocd

rx: Lithium 1200 mg, Lyrica 600 mg, Vyvanse 60 mg, Latuda 40 mg, Viibryd 40 mg

It might be withdrawal. That is the obvious explanation, of course. You could be experiencing temporary withdrawal rebound depression / anxiety that will dissipate over time. Alternatively, it could be a side effect of Viibryd. I found Viibryd to produce anxiety for much of the time I took it. It reached a peak in severity during week 3. If it were me, I would restart the clomipramine and wait to see what Viibryd does for you. If you are happy with what you are feeling, you can then perform a very gradual taper of the clomipramine. You might need both drugs.

Hi Eric.

> You might need both drugs.

I guess not.

I misunderstood what you were describing. You are not looking to feel any better on Viibryd than you did on clomipramine - just no worse.

Was depression or anxiety ever a problem for you?

Scott asked Eric this in another thread. I remembered you wrote that you experienced anxiety and normally don't have it. Would this mean that since OCD is in the anxiety spectrum this wouldn't be a good med then? Could anxiety in your case have been a lessening of depression this thought just this second popped into my mind? Phillipa

Good questions.

I am concerned that Viibryd can exacerbate anxiety or OCD via stimulation of postsynaptic serotonin 5-HT1a receptors. This is just theoretical, though. Time will sort this out.

For me, increased anxiety is not associated with a decrease in depression. I must use mental gymnastics to control anxiety, especially in social situations. Anxiety was more of a problem when I was younger.

Yes I remember you also posted this about anxiety. So even stranger could this mean depression is heading back to where it began? Before treatment resistant? Grabbing at straws. Phillipa

What was happening was my mood was becoming unstable. I was having mixed symptoms and in my case, that carries a lot of anxiety with it. I nipped it in the bud by increasing my Latuda. I'm feeling much better now.

As it turns out, my mood had become unstable toward the end of my taper from the Clomipramine to the Viibryd. The past week I have been having mixed symptoms: depression, anxiety and irritability. This tends to happen with me on SSRIs. I tackled the issue by upping my Latuda to 80 mg, which has calmed things down greatly.

Smart.

I hope Viibryd does the trick.
